Tribute Gift
Here’s your opportunity to make a donation in honor of a friend or relative or someone important to you. When you make a tribute gift, we ask for your name, address and the honoree’s name and address. We’ll send a card to the person you wish to be notified of your gift. You’ll also receive a card from HDSA to confirm your donation.

Memorial Gift
A meaningful way to honor the memory of a loved one. We ask you to fill in your name and address, the deceased person’s name, and the name and address of the person to whom you want us to send an acknowledgment (to you or to the person you designate as the recipient). We’ll send a card to the person you wish to be notified of your gift. You’ll also receive a card from HDSA to confirm your donation.
